Community Care Cooperative (C3) Delivers $5.1 Million in Savings to Its ACO REACH Model Partners in 2023 and Expands Its Program in Massachusetts and Washington State

BOSTON--(BUSINESS WIRE)--Community Care Cooperative (C3), the only Accountable Care Organization (ACO) in Massachusetts founded and governed by Federally Qualified Health Centers (FQHCs), announced that it achieved $5.1 million in shared savings during the 2023 performance year of the ACO Realizing Equity, Access, and Community Health (ACO REACH) Model, according to data recently released from the Centers for Medicare and Medicaid Services (CMS).

This also includes an impressive $11.5 million in savings for Medicare, underscoring C3's commitment to delivering high-quality, cost-effective care. C3 shares in the risk and reward across 43 FQHCs’ value-based agreements, representing more than 45,000 Medicare beneficiaries.

FQHCs are known for delivering high-quality, person-centered care that reduces total medical expenditures. C3’s partners are committed to sustaining local primary care by improving outcomes, quality of care, and cost-efficiency.

C3 is also pleased to welcome eight new participating partners from Massachusetts and Washington to its ACO REACH Model. This expansion extends C3’s network to over 13,000 additional Medicare beneficiaries, furthering our commitment to the provision of high-quality, value-based care.

The eight new participating FQHCs are:

  • Caring Health Center, MA
  • Community Health Care, WA
  • Community Health Center of Snohomish County, WA
  • Cowlitz Family Health Center, WA
  • HealthPoint, WA
  • Lawrence Family Doctors, MA
  • Upham’s Corner Health Center, MA
  • Yakima Neighborhood Health Services, WA
